Job Description :

• Support with CMS Star Improvement program including but not limited to

Responsibilities:

• Outreach to Pharmacy’s to support the Pharmacy teams outreach efforts and close the gaps

• Outreach to members across Medicare business lines to close gaps in care

• Respond to any follow-up calls from members or providers on the outreach effort

• Support members with scheduling PCP appointments and if necessary, other specialist appointments (as requested)

• Support with mail merges for select quality measures

• Manage outreach spreadsheets – clearly documenting efforts and reporting to management on a regular basis

• Conduct Star MTM program outreach

• Other Star-related and administrative responsibilities as deemed necessary

Experience:

• 2-4 years of experience preferably in Healthcare or managed care or Business related field .

• Skilled in MS Word, Excel, Powerpoint etc.

Skills:

• Good Communication skills

• Strong attention to detail

• Multitasker and ability to work within a team environment

• Strong experience with MS Excel

Education:

Bachelors degree or equivalent experience
