Trinity Health Peer Specialist - Mobile Crisis Outreach in Syracuse, New York

A valid NYS Drivers license and clean driving record is required

The Mobile Crisis Outreach (MCO) program provides services to individuals experiencing mental health crises in the community. They complete scheduled follow up visits to assess treatment compliance and facilitate community connections and referrals for patients recently discharged from CPEP or the St. Joseph’s Health inpatient psychiatric unit.

MCO also participates in the 988 Crisis Response Program, taking referral calls from the national crisis line or as part of the 911 Diversion Program. The team responds immediately to offer help to individuals in the community when they need it most.

A Peer Specialist in the MCO team will work as part of an interdisciplinary team of Registered Nurses, Licensed Social Workers, Peer Specialists and Patient Care Techs. They will serve as a person's individual advocate, assist them to meet their treatment goals and assure that the person's choices drive their goals and plans.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:

  • High school diploma or equivalent (GED) required

  • Must be a Certified Peer Specialist or have provisional certification through NY York Peer Specialists (NYCPS) or The New York Peer Specialist Certification Board (NYPSCB)

  • Current, unrestricted New York State Driver’s License

  • Identify themself as a current or former user of mental health or co-occurring services.

  • Must have an understanding of recovery concepts and person-centered approaches and be successfully implementing them in their own recovery.

  • Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise. Completes population specific competency annually on populations served as identified in scope of care and services.

  • Maintain BLS/CPR certification. Maintain other certifications appropriate to field.

Supervised By:

  • Program Coordinator, Unit Manager and Director.

Pay Range: $18.50 - $26.85

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
